Please download and print copies of the Donation Form for your team members. Each member should enter donations received on this form, along with the donor's information. Checks are payable to: The Cam Neely Foundation.


Race Entry Forms must be postmarked by Thursday, April 19th 2007. Make sure team members each receive a Donation Form to begin gathering donations by race day. Remember, the TOP 3 teams with the highest donations received are eligible for prizes! Good luck.
